Starting IVF can feel overwhelming, especially when you’re faced with new terms, medical procedures, and a rollercoaster of emotions. That’s why understanding the IVF process is so important. Knowing each stage — from ovarian stimulation and egg retrieval to fertilisation, embryo transfer, and follow-up — helps you feel more prepared, reduces uncertainty, and gives you greater confidence in your treatment journey. How Does IVF Work?

Understanding the IVF process is an important step in feeling confident and supported as you move forward. In vitro fertilisation is one of the most trusted and effective fertility treatments available. The process involves stimulating the ovaries to produce eggs, carefully retrieving them, and then combining eggs and sperm in a lab to create embryos. These embryos are closely monitored, and one or more are then gently transferred into the uterus to help achieve pregnancy.

Through this process, IVF provides couples with a chance to conceive a child despite certain fertility challenges. With careful monitoring and precise handling of the eggs and sperm, the success rates of IVF have continued to improve over the years, making it a popular and effective treatment option for couples seeking to start a family.

In vitro fertilisation stages involve a series of carefully managed steps that work together to support the chance of pregnancy.

Step 1: Initial Consultation – Fertility Assessment

Your fertility journey begins with a comprehensive initial assessment conducted by our experienced specialists. We recommend attending this appointment with your partner, if applicable, to facilitate joint evaluation and discussion of treatment options. We also warmly welcome single women, same-sex female couples, and individuals or couples seeking fertility support.

During this consultation, your specialist will review your medical history and perform baseline investigations, including blood tests, ultrasound imaging, and semen analysis (if applicable). These assessments include:

1. Blood tests for antenatal screening, ovarian reserve (AMH), hormonal profile, and chromosomal analysis.

2. Pelvic ultrasound to evaluate reproductive organ structure and identify potential issues.

3. Semen analysis to assess sperm quality, motility, and morphology.​

Step 2: Pre-Treatment Planning 

Following the initial assessment, you will return for a pre-treatment consultation to review test results and develop a personalised treatment plan. This includes signing consent forms and scheduling medication protocols. A nurse orientation session will also be arranged, during which your fertility nurse will guide you through medication administration, including FSH injections, and provide a treatment timeline to clarify key milestones.

Step 3: Ovarian Stimulation

Ovarian stimulation aims to induce the development of multiple mature eggs, increasing the likelihood of successful fertilisation. This process involves daily hormone injections, typically FSH and GnRH antagonists, tailored to your response. Close monitoring through blood tests and ultrasounds ensures optimal follicular development and minimises risks such as ovarian hyperstimulation syndrome (OHSS).

Monitoring includes blood tests measuring estradiol and progesterone levels as well as transvaginal ultrasounds to assess follicle size and number, as well as endometrial thickness.

Adjustments to medication dosages are made based on ongoing assessments to optimise outcomes and ensure safety.​

Step 4: Trigger Injection and Egg Retrieval

When follicles reach the appropriate size, a trigger injection of hCG or GnRH agonist is administered to mature the eggs and time ovulation. Egg retrieval is scheduled approximately 36 hours post-injection, performed under sedation or anaesthesia, with ultrasound guidance to aspirate eggs from the ovaries. After the procedure, patients rest in our recovery area, and the number of eggs retrieved is communicated to you. The eggs are then prepared for fertilisation.

Step 5: Sperm Collection and Fertilisation

Sperm collection, usually on the same day as egg retrieval, can be achieved via masturbation or alternative methods if necessary. The sample undergoes analysis and preparation to select the most viable sperm for fertilization, which can be performed through conventional insemination or ICSI, depending on sperm quality and previous treatment history.

Step 6: Sperm Collection and Fertilisation

Fertilised eggs develop into embryos over five to six days in a controlled laboratory environment. Embryologists monitor growth and select the best quality and most viable embryos for transfer or cryopreservation. Embryo quality assessment guides these decisions, aiming to maximise pregnancy success.

Step 7: Sperm Collection and Fertilisation

The embryo transfer procedure involves placing the selected embryo into your uterus via a thin catheter under ultrasound guidance. This minimally invasive process is typically painless and does not require anaesthesia. Post-transfer, patients are advised to rest and avoid vigorous activity. The success of implantation is monitored through hormone support and pregnancy testing.

Step 8: The Two-Week Wait and Pregnancy Testing 

Following embryo transfer, a two-week period ensues before a blood test measures hCG levels to confirm pregnancy. This period can evoke emotional responses; support services are available. Patients are advised to adhere to medication protocols and avoid early home pregnancy testing to prevent unnecessary anxiety.

Step 9: Embryo Cryopreservation and Frozen Embryo Transfer

Remaining high-quality embryos can be cryopreserved via vitrification for future use. Frozen embryos are stored securely and can be thawed for subsequent transfer cycles, offering flexibility and reducing the need for repeated ovarian stimulation. FET procedures involve preparing the uterine lining and transferring thawed embryos, with success rates comparable to fresh cycles.
